I have one month to spend in an elective in the US. I have no specific direction in medicine right now. I just want to get a glimpse at how high-quality american medicine is like. At which university/hospital/department do you think it would be best to do that? Keep in mind that having fun and having reasonable working conditions is very important to me too.

Do something in a city like Boston or New York, and rotate on lighter services in surgery where you don't need to take call, like plastics. Also do a program where there are fellows but not too many visitors and medical students so you actually get to see things, but not one that's so busy where you are working like an intern.

The ER is as close to primary care as the urban US has these days. Plus, the rotation schedule is usually pretty light, and you take NO call. ER docs tend to be laid back and very open about discussing the positives and negatives of the American medical system.

isn't it a bit hectic at the ER? I guess it depends on the hospital and on the neighborhoud. But I don't know the US very well.. I guess I might like New york, Boston or San Fransisco.. are hospitals in these places variable in terms of quality and population? I'm talking of course only about the university-affiliated hospitals where you can do electives..
