How competitive are year long derm research fellowships?

How competitive is it to get a spot in a one year research program? i am considering this between 3rd and 4th year. I know that not all derm residency applicants take a year off, so will doing so boost an otherwise not amazing application or do i still need to be just as qualified (grades/boards/prior research) as applicants who go straight to residency?

I think that most of the fellowships that people are going after call for medical school grads, and often even people with a license.

People that I met on the interview trail who took a year off between 3rd and 4th year got a non-derm related fellowship, and did them in derm labs. Dorris Duke, Howard Hughes, etc. Apply for med student fellowships, and get a hold of derm labs to do research in.
