Be willing to make less money, and seek out those opportunities. Don't put yourself in a situation where other peoples' salaries depend on your clinical productivity. Kids go to bed, you can work when they're asleep. In a big group or academic situation you don't have to work a ton of weekends. Take on administrative roles that can limit your clinical exposure. Funded administrative effort is more flexible and often more efficient than clinical effort, even if it is reimbursed less extravagantly.

Alternatively, become a well-funded investigator. You might work a lot of hours, but the could be more flexible. But you'll make a lot less.
