I can't give you exact % or salary amounts, but there are people who do a transfusion med fellowship and then go into private practice. In general, any fellowship you do will make you more competitive for a private practice, because you will have more expertise. You don't need a transfusion med fellowship for private practice though, although there may be positions out there that require it. If people are going to do fellowships and go into private practice, they often do one that will give them a wider range of job opportunities, like cytopath or surg path. The world needs blood bankers, though. You could do cytopath + blood bank and probably name your future location.
I suppose you could always be angling for a job with the red cross. Or, you could be Nosferatu.
