When to start asking talking to a hospital about a neurocritical care job

I'm one of the very happy neurology residents who matched into a neurocritical care fellowship and I know a few hospitals that I'd really like to work at in 3 years once I'm done. When should I approach them about opportunities?

Is it too early?

I'm sure you could contact them and let them know your interest. Hospitals always like speaking with potential recruits. However you realize they are probably in the position to fill their positions as soon as possible. So while you might be interested when you finish in 3 years, things can change over that time in regards to what positions are open. When you are serious about potential jobs after your fellowship, I would wait until you are into your last year of the fellowship.
 
It's always nice to maintain contact with places you have an interest in working. However, funding for positions is not worked out that far in advance at most academic institutions, and job vacancies are not typically known until someone tenders a resignation. As a result, there is little that anyone can do with the information. Heck, I'm a senior neuroICU fellow looking for a job, and several academic positions that were supposed to come available for 2012 have not, due to the economy and vagaries of intra-department politics at some institutions. Trying to plan for 3 years in the future is damn near impossible.

But, again, it's a small world and maintaining connections can't hurt.
