Okay.... I have a few questions as a DO interested in Radiology just starting to investigate the MD world.

1. Do MD's do a traditional year, then apply for their rad spots or are they linked into the residency and then find a traditional year??

2. If I were to do a DO traditional year, could I try and find a PGY-2 MD spot?

3. Is the MD Rads world really all about the scores?? Anyone out there involved with the interview process that would truly know?? Do they just line us up by boards and count down to however many they wish to interview??

The majority of MD rads programs are advanced, meaning you match into the PGY2 year (the first year of rads) in your 4th year of school. A minority, not sure how many, I'd guess a couple dozen, are categorical - this means the transitional year is included. You match into PGY1 in addition to the rads residency and complete them at the same place.

The presence of advanced matching in the NRMP is one of it's great advantages over the AOA match. Nobody likes going into an internship and waiting a year to find out if they're even going to wind up in the specialty they want (which is the case with most DO specialties).

You have several options.

1. You can apply to both categorical and advanced MD rads programs. If you match into a categorical, you're all set PGY1-5. If you match into an advanced program, you scramble into a) an MD transitional year b) an MD prelim year in IM or Surg or c) a DO rotating internship. Doing the DO rotating internship would require approval from your residency program that they will accept it as PGY1.

2. You can apply to DO rotating internships through the AOA match, and then apply only to the advanced MD rads programs (once you match into a rotating internship in the AOA match you'll get pulled from the NRMP PGY1 match, voiding you for categorical programs).

3. You can apply to MD rads advanced programs as well as MD transitional/prelims. You fill out a ranklist for both. You wouldn't be able to apply to the categoricals because they would conflict with your transitional/prelims (both PGY1). ***EDIT: See correction in post below***

Option 1 gives you the best chance of matching, and I believe is what most people do. Hope this answers your questions.

nicedream more or less has it down, except for number 3. On the rank lists you specifically arrange prelims/transitionals for each advanced program. For categoricals, there is simply no prelim list. It's not a problem to apply to categorical and advanced programs as well as any PGY-1 years that you want.

How do I determine which of the programs are PGY 1-5 or 2-5? To be honest, I don't think my scores are worthy of a MD Rads residency, but I would like to be armed with the information.

In FREIDA, if you go under Optional Criteria, you can select "does not require previous GME." I'm not sure if this catches all of them though - it comes up with 15 programs. Best bet is checking the program websites.

The only addition would be categorical programs. You just don't make a sublist for them.

The match can be annoying as a DO. One of my favorite programs was categorical, so I didn't participate in the DO intern match at all. At the same time, I didn't want to scramble into some leftover PGY-1 year. So now I'm in an allo prelim year and I'll have to petition the AOA and all that crap if I want to practice in the five states with the internship rule.

It was my impression that there were plenty of AOA internships left over after match, not necessarily the more undesirable ones. No?
