In the http://www.nrmp.org/2004advdata.pdf one can see that 2 surgical spots were unfilled in 2004, 3 unfilled spots in neurological surgery, 18 in anasthesiology, and 7 in emergy med. Which are most likely the underlying causes for these empty spots?

Both surgical and and neurological surgical recidencys are to my knowledge highly popular areas.
 
A friend of mine grabbed one of the g.surg spots and it was a community program in a small midwestern area, probably not too desireable to most. Programs occassionaly don't make their lists long enough.

Those are probably pre-scramble spots. Trust me, by 10 am the following morning none of those spots were unfilled.
